Most rhinoplasty candidates seek treatment because they do not like the side view of their faces. Indeed, the shape of the nose plays a big role in the side view of the face. In some individuals, the changes to the bridge, length and angle of their nose create a new face perception when viewed from the side.
The shot in which the human face is precisely photographed from the side at a ninety-degree angle is called a “profile view”. In the profile view, the facial contour begins with the forehead at the top and continues down with the nose, lips, chin and neck contour. The nose contour is a sub-unit of the facial contour and is closely related to other sub-units in terms of aesthetics.
When designing the side view of the nose for rhinoplasty, we also take into account the patient's forehead, lip and chin structure. The same nose will look different on a face with a short/round forehead than a face having a long/flat forehead. Similarly, the harmony of the same nose with a small and receding jawline is different from that with a large and protruding jawline. Therefore, we often combine operations that change the contour of the forehead and chin with rhinoplasty.
In individuals with a flat forehead, giving a more rounded look to the forehead with fat injections is a procedure that we often combine with rhinoplasty. When we give the forehead a more rounded appearance, a nice curvature occurs between the nose and forehead. This procedure provides a significant visual benefit, especially in cases where the forehead-nose transition is straight, and the nose looks as if it starts directly from the forehead. The visual enlargement of the forehead creates the illusion of a relatively smaller "nose". This effect is also very valuable, because we need to preserve the dimensions of the nose as much as possible in order to preserve the breathing function.
Another procedure that we often combine with rhinoplasty is brow lift/endoscopic forehead lift. The very low and flat structure of the eyebrows leads to a "masculine" appearance on the face. Both a large nose and low, flat eyebrows can mask the beauty of the eyes in female patients. The main advantage in combining these two procedures is that the eyes suddenly appear more striking after the operation.
In individuals with a receding jawline, chin prosthesis or mandibular advancement surgery (genioplasty) can be performed simultaneously with rhinoplasty. The combination of chin surgery and rhinoplasty has the power to transform the profile view of the face very effectively. A weak chin structure makes the nose appear larger than the face overall. When a high nose bridge and a receding chin are combined, the face assumes a convex appearance in the profile view. Therefore, the combination of rhinoplasty and chin advancement surgery is much more effective, particularly in male patients with a receding chin and a high nasal arch.
In summary, forehead and chin contour should be taken into consideration during the planning of rhinoplasty. It is possible to perform these procedures in combination in the same anesthesiological session. On the other hand, it is not compulsory to perform these procedures simultaneously. They can be done in different sessions as well.
