Who Is Ineligible For Deep Plane Facelift?

There are various surgical techniques in facelift.

From time to time, certain techniques gain popularity in the hands of successful practitioners, particularly when the media gives them wings.

Nowadays, deep plane facelift is very popular.

However, we should keep in mind that no surgical technique always yields good results in the hands of any practitioner and on every face.

Each technique has its own indications (do it) and contraindications (do not do it).

In the hands of the right practitioner and coupled with accurate patient selection, deep plane facelift produces very permanent, effective and beautiful results.

But not everyone can do it, and not everyone is eligible for it.

First of all, let me remind you once again what I mean when I talk about "deep plane".

Deep Plane = Deep Surgical Dissection Plane of the Face

Deep in the SMAS layer, this dissection plane is just above the nerves and muscles that move the face.

Surgical errors in this plane may result in facial paralysis.

It's no joke.

A surgeon who has not received "adequate training" in the surgery of this area should not be lured into thinking "I will learn as I do".

Deep plane anatomy differs on each individual's face, even between the right and left halves of the same individual's face. What I mean by adequate training is to have the experience of observing as many surgeries performed by other surgeons as possible to allow for a full grasp of these anatomical differences and to satisfy the need to make changes in the action plan accordingly as well as the experience of performing such surgical procedures under the supervision of another surgeon.

In order to secure an effective result in deep plane facelift surgery, it is necessary to surgically mobilize all the retaining ligaments that hold the face in place. In some areas, the distance between these retaining ligaments and facial nerves may decrease to less than 1 mm. If you will not be able to mobilize all retaining ligaments, it would be better not to prefer deep plane facelift surgery initially. The reason is that a duly-performed superficial facelift is both more effective and safer than a sloppy deep plane facelift.

In deep plane facelift surgery, this anatomy must be "virgin" in order to achieve a "complete" mobilization at the level I mentioned above.

All cases where the ideal anatomy is disrupted in deep plane facelift surgery are relative contraindications.

Reviewing their medical history, the following patients should be ruled ineligible for a deep plane facelift:

  • Patients who have received permanent synthetic facial filler injections,

  • Patients who have received smart filler injections that create scars in the face (increasing collagen),

  • Patients who have received permanent thread lift inside the face,

  • Patients who have received liposuction inside the face,

  • Patients who have undergone a facelift surgery the details of which are not fully known,

  • Patients whose main salivary gland (parotid) has been surgically removed,

  • Patients who have received facial radiotherapy,

  • Patients who have been treated for infection or abscess in the facelift area,

  • Patients with vascular malformations in the facelift area.

 

 

So it can't be performed at all? Is it impossible?

Maybe you can push the limits of the surgical technique to do it, but you will take unnecessary risks for both your patient and you.

We always question these risk factors preoperatively in medical history forms. If the patient unconsciously or consciously (It happens, believe me!) has given us insufficient information about the risk factors above and we have noticed this during surgery, we change the operation plane from deep to superficial.

Particularly some fillers done in the past may not be noticed in the preoperative examination unless the patient tells. Everything looks normal from the outside but you may encounter a hard tissue mass that does not allow surgical dissection.

In such cases, it may be necessary to finish the surgery within the limits of "mini facelift" for patient safety.

Furthermore, there is a group of patients who must not undergo facelift surgery "no matter what technique is employed".

In addition to hundreds of different organic causes such as organ failures and uncontrolled coagulation disorders, dozens of different psychiatric disorders such as body perception disorder, symmetry obsession and personality disorders can also set an obstacle to facelift surgery.

The patient may not be aware of these.

Therefore, facelift surgery is not a "product" that we may choose from an online catalog, but a medical treatment that can only be recommended to us by a professional.
